In-Home Hospice Care Across Texas

From Houston to Dallas, San Antonio, Austin, El Paso and the Rio Grande Valley, our hospice nurses come to your home and our live-in carers stay by your loved one's side.

How it works

Prime Choice Hospice & Palliative Care is a Houston-based hospice that provides in-home hospice care throughout Texas. Our own hospice nurses travel to patients' homes on a schedule set by the care plan. A live-in carer can stay in the home between visits, and a nurse is on call 24/7. For eligible patients, Medicare and Texas Medicaid cover hospice the same way everywhere in the state.

Wherever You Live in Texas

How Does Our Hospice Care Reach Homes Across Texas?

Traveling Hospice Nurses

Prime Choice registered nurses travel to your home to assess symptoms, adjust medications and teach your family, on the schedule in your care plan.

Live-In Carer

A carer who stays in the home helps with bathing, meals, repositioning and companionship, and alerts our nurse to any change.

24/7 Nurse Line

Day or night, one call to 832-622-6418 reaches a hospice nurse who knows your loved one's plan.

Physician Oversight

Our hospice physician directs the plan of care with your loved one's own doctor, and arranges medications and equipment for the home.

What Is Live-In Hospice Care?

Most hospice programs visit on a schedule and leave the round-the-clock caregiving to the family. For many Texas families, especially adult children who live far away or a spouse who can't lift or stay awake all night, that's the hardest part of end of life care at home.

Prime Choice can place a live-in carer in the home as part of your loved one's hospice team. The carer:

  • Helps with bathing, grooming, toileting and changing linens
  • Prepares meals and helps with eating and drinking as appetite changes
  • Repositions your loved one to prevent pressure sores
  • Provides companionship, so your loved one is never alone
  • Calls our hospice nurse right away when something changes

Depending on your loved one's needs, live-in support may be included in your hospice care or arranged privately. We explain exactly what's included, and any cost, before care begins.

What Does Hospice Cover in Texas?

The Medicare hospice benefit is federal, so coverage is the same in every Texas county. For the terminal illness it covers nurse visits, physician oversight, hospice aide visits, social work and chaplain support, comfort medications, a hospital bed, oxygen and other equipment and supplies. The only possible costs are up to $5 per prescription and 5% of the cost of inpatient respite care. Texas Medicaid covers hospice through the HHSC Medicaid Hospice Program, and most private insurance and VA plans include a hospice benefit.
